Abdominal Transplant
Tulane Abdominal Transplant Unit (kidney, kidney/pancreas, liver, and small bowel) is the first of it’s kind in the country. This unit consists of 18 beds, all private, that provide total care for abdominal transplant patients in all phases, both pre and post transplant. This unit appeals to the nurse that is looking for a service-focused unit. The population and acuity vary between critical to ambulatory. It further allows nurses that are interested in transplant to sharpen their skills in an academic environment.

The National Institutes of Health recently published that Cancer has replaced heart disease as the number one killer of Americans. Every person will be touched by the diagnosis of cancer in their lifetime, whether the individual, a family member, or friend.

Also, our cancer center is an approved American College of Surgeons facility. This is a significant accomplishment because only 35 percent of the existing cancer programs throughout the country are so designated.

At the Tulane Cancer Center our team of professionals share a common focus and goal, the improvement of health and quality of life for out patients, and ultimately the prevention and cure for cancer.

Cardiology
I. Invasive Cardio-Vascular Lab
is a M-F dept., closed weekends and holidays. Call required. Critical Care, Cath Lab, or Special Procedure Lab experience required; BLS and ACLS required, PALS a plus. RN, RRT, CVT, or RCIS required. Services include Adult Cardiac Cath, Pediatric Cardiac Cath, Electrophysiology, and Special Procedures/Interventional Radiology.

II. Cardiovascular Recovery Area
is a M-F dept., closed weekends and holidays, no call. Cardiac experience preferred, BLS and ACLS required. 8 bed monitored unit, works closely with cath lab and special
procedures. Services include: pre/post care of cardiovascular patients, cardiac infusions, assisting with cardiac procedures to include TEE's and cardioversions.

III. Noninvasive Cardiology and Cardiac Imaging Centerare
is a M-F dept., closed weekends and holidays. Call is required. Services include ECG, ECHO, stress test, cardiac nuclear and peripheral vascular studies. Registration, certification and experience requirements depend on imaging modality.

IV. Cardiac Transplant/Advanced Heart Failure Dept.
is a M-F dept. closed weekends and holidays. Call is required. Extensive experience with managing complex cardiac patients is preferred. BSN required, NP preferred.

Downtown Clinics
Tulane Multispecialty Clinic - Downtown is an adult primary care practice as well as medicine specialties. While the practice delivers care to many chronically ill patients, it has heavy emphasis on optimizing health promotion.

Note: Both the adult and pediatric clinics serve as practices for the following specialties: Allergy, Immunology, Rheumotolgy, Pulmonary, Nephrology, Endocrine, Gastroenterology, Hemophilia and Infectious Disease.

Metairie Clinic is located on the Lakeside campus in suite110 of the 4770 medical office building. This clinic offers both primary and multi-specialty medical care to both adults and children.  This practice includes specialist in general internal medicine, orthopedics, ENT, and audiology.

Orthopaedic Clinic provides evaluation and management of the adult and pediatric patient with a variety of orthopaedic conditions.

Ophthalmology Clinic evaluates and manages the care of both adult and pediatric patient.

Surgery Practice Located at the Lakeside campus in suite 309.  This is an adult and pediatric general and specialty surgical practice.  Specialties include, Vascular, Pediatrics, Plastic-Reconstructive, Craniofacial and Breast surgery.  We a have a complete Vascular Diagnostic and Breast Diagnostic Centers within the suite to offer excellent and efficient patient care.

Dermatology Practice is located at the Lakeside campus in suite 202.  This practice evaluates and manages both adults and children with dermatological conditions.  We also offer cosmetic dermatologic services and Phototherapy services.

Cardiology Clinic evaluates and manages the care of the adult with cardiac conditions, which are medically managed. Children receive evaluation and management in the Pediatric clinic.

Otolaryngology Clinic evaluates, manages, and offers diagnostic studies for a variety of conditions of the ear, nose, and throat. Both adults and children receive care in this practice.

Urology/Fertility Clinic evaluates and manages conditions related to the renal system, which may be either acute or chronic, in both the adult and child. The practice also evaluates and manages the care of both male and female patients with conditions related to infertility. Diagnostic studies and procedures related to the renal system and infertility are performed in this practice.

Satellite Clinics
Tulane Multispeciality Clinic at Uptown Square promotes optimal health and evaluates and manages the care of acute and chronic conditions of the adult and child. It is located in Uptown New Orleans at 200 Broadway.

Metairie Clinic offers both primary and multi-specialty medical care to both the adult and child. This practice is adjacent to Lakeside Hospital.

The Institute of Sports Medicine is located on the Tulane University campus in the Reilly Center. This practice primarily evaluates and manages orthopedic care to patients who require treatment as a result of a sports injury. Diagnostic radiological studies and physical therapy are offered at this location.

Critical Care
Tulane is a tertiary care facility, and the Critical Care units are challenging environments to work in. You will have the opportunity to work with a variety of patients and have an opportunity to master various technical skills in a learning environment.
SICU : 13 bed surgical intensive care unit. Nurse: Patient ratio is 1:2, 1:3 or 1:1. Types of patients: CABG's, Valves, craniotomies, Carotid Endartarectomies, Femoral Popliteal Bypass, Lumbar Fusions, Pelvic Fracture repairs, Gastric Bypass, etc.
CCU/MICU : an 11 bed combination unit of coronary care and medical intensive care patients. Nurse: Patient ratio is 1:2, 1:3 or 1:1. Types of patients:acute MI's, PTCA's, respiratory distress, GI bleeds, Tracheal Stents DKA's, etc....
BMTU: a 7 bed Hematology/Oncology unit. Nurse: Patient ratio is 1:2, 1:3 or 1:1.

ED
We are a 16 bed main emergency department that sees approx 31,000 patients a year. The main department is staffed by Board Certified Emergency physicians only. The emergency department does not have a residency program, and residents do not moonlight in the ER. The main department has 2 physician coverage 16 hours a day, and one physician coverage for the remaining 8. Tulane University abdominal transplant service has grown to be among the largest in the nation and the ED is a portal to a newly redesigned Transplant Unit. The Emergency Department dedicates 3 beds to isolation purposes if needed. New Orleans’ patient population consists of high acuity medical patients, most with many co-morbid illnesses. In addition, we have a significant amount of trauma mainly composed of motor vehicle accidents, stabbing's, overdoses and gunshot wounds.

We have a single large triage area and a dedicated triage nurse 24 hours a day. A dedicated medical screening room used for minor care or to fast track patients to cath lab or OR and are on our way to becoming a chest pain center. Additionally, we have a charge nurse that does not maintain patient assignments, and is free to float through the department and assist with patient care. We attempt to maintain a 4 to 1 nurse to patient ratio with emergency room patients aside from the charge nurse and the triage nurse adding additional Med-Surg and ICU. We are located within minutes of the French Quarter and service a high number of tourists.

Lab
The laboratory is comprised of five separate and distinct sections. Since this is a teaching hospital, specialists in every area of the lab are required to adequately meet the testing needs and interpretations of testing for our tertiary patients. The five sections are General Laboratory, Chematology, Specialty Laboratory, Blood Bank, and Anatomic Pathology. Keeping our equipment on the cutting edge of technology is a main thrust of our laboratory, and “Keeping Knowledgeable through Change” is our motto.

Lab Assistants tend to gather a wealth of knowledge and because of our affiliation with the Med School; they have the chance to process various kinds of specimens. We offer the opportunity to learn from experience. A motivational rewards program is functioning and well received.

Opportunities for career development are available for all staff. Tuition reimbursement and, in some cases, outright grant money is available. Masters Degrees are available through the Tulane School of Public Health. MLT to MT programs are available. Tulane University Laboratory provides clinical training to the local MLT students and for local phlebotomy programs.

Perioperative Services
Operating Room - staff 4 operating rooms including cysto and lithotripsy rooms.
Staffed with both RN’s and OR technicians and provides services for all specialties including open heart and transplant.

Staffed from 7a - 7p five days a week with on-call coverage for after hours, weekends and holidays.

ACLS/PALS required for Outpatient Surgery - 18 private rooms with 2 minor procedure rooms including pre-admit testing. Staffed with all RN’s and secretary. Opened 5 days a week 5:30a - 11p. Closed weekends and holidays. ACLS/PALS required Post Anesthesia Care Unit - 10 beds, including one isolation room, staffed with all RN’s. Two to One ratio for majority of patients except One to One for kidney transplants and ICU holds. Hours of operation are 7:30a - 9p, with on-call coverage after hours, weekends and holidays. Inpatient dialysis is currently performed in PACU.

ACLS/PALS required for Endoscopy - Staffed by RN, one LPN and Endoscopy tech’s - only inpatient procedures are performed.

ACLS/PALS required Central Sterile - provides support for all nursing units and clinics. Processes all instruments for OR. Staffed with Central Sterile technicians 24 hours a day Monday through Friday, 7a – 11p, Saturday and Sunday, covered on-call. Presently located within the OR.

Radiology
Extensive primary and specialty services, along with a mix of pediatric, adult, and geriatric patients can be found at our hospital based teaching facility. We provide world-class healthcare in a community setting. The friendly staff of Tulane Department of Radiology provides an atmosphere of genuine caring for patients and staff alike.

Diagnostic Radiology
A dynamic team of qualified professionals staff the department, which consists of a digital chest room, two digital fluoroscopy suites, panelipse unit and three general diagnostic machines in the main department. We also have a unit in the Emergency department as well a three located in the orthopedic clinic.

Ultrasound
Two full-time technologically sound imaging specialists staff the three Acuson Sequoias. The department provides coverage to our cutting edge abdominal transplant program.

CT

The department is staffed 24/7 with back up on-call providing coverage to our busy Emergency Department and Abdominal Transplant Programs. State of the art 3-D imaging provides quality exams to our Specialty Orthopedic surgeons.

MRI
A 1.5 tesla magnet is the mainstay of the department. The highly skilled staff loves the challenge of performing and participating in complex studies and research projects initiated by this teaching facility.

Mammography

With state of the art Computer Aided Detection, our licensed and registered Mammographers are dedicated to providing the highest quality care available.

Nuclear Medicine
The licensed and registered technologists that staff out nuclear medicine department perform cutting edge diagnostic testing.

Inpatient Rehabilitation Center
The Tulane Inpatient Rehabilitation Center is a comprehensive Rehabilitation program designed to restore patients with various physical, occupational, and speech disabilities to a maximum level of independence as quickly as possible. The schedule of therapy is appropriate to patients’ needs while meeting all regulatory requirements. In 2004, our center exceeds the regional and national rates for functional improvement and overall efficiency of improvements made. The Tulane Rehabilitation Center discharges patients back to the community at a significantly higher rate than the average rate of rehabilitation units in the region and nation.

I. Staffing

Tulane Medical Center is a teaching facility. This means that our case load is usually very diverse. The cases that we see can be very straight-forward textbook situations or can be extremely complex medical/surgical/orthopedic situations that require us to be creative in devising unique plans of care. This is made easier by the high level of expertise that exists within the department. This level of expertise is complimented by the diverse geographical basis of the staff’s experiences.

II. The WorkLoad:
Staff see between 8 and 15 patients per day – some QD and some BID. Most patients are seen bedside. Patients can be brought to the Gym or Whirlpool room as needed.

Occasional input to Vascular Clinic and Dermatology Clinic

All Physical Therapy positions are exempt status. The work day is scheduled as 8am to 4:30pm with 1 D2 hour lunch. However there is some flexibility. There is always one person scheduled 8:30am to 5pm to cover late Outpatient surgery patients. The staff are a close group. Because PT staff are exempt status the work day actually runs until the work is complete. The staff all stay and help each other until all the work is done.

Weekend coverage is provided by a mixture of full time and prn staff.
On Saturday there are 2 physical therapists. There is a priority list of patients and staff work until all of those patients are seen. Full time staff may be required to work 1 or 2 weekend days per month – sometimes more and sometimes less. When a full time member of staff works a weekend day, he/she takes a day off during the week.

The PT department works closely with the Inpatient OT department to provide a team approach to patient care. We also work closely with Outpatient PT/OT providing continuity of care after discharge.

As part of a teaching facility, we take students for placements from a variety of schools. Some students come for two week units and some come for as much a 8 to 10 weeks at a time. All staff are encouraged to participate in the student training program and although each student has a Clinical Instructor, all staff usually interact with the students to share their expertise.

Therapy Services—OT
Occupational Therapy services are provided for Inpatient Acute care at Tulane Medical Center downtown and Tulane-Lakeside, Tulane Inpat. Rehabilitation Center at Tulane-Lakeside, and at the outpatient clinics downtown and on the Tulane-Lakeside campuses.

Occupational Therapy services at Tulane includes: inpatient and outpatient pediatrics, NICU/PICU, adult acute care, outpatient adults and hand therapy, vestibular rehabilitation. We assist with a variety of clinics including wheelchair/seating clinic, spasticity and feeding clinics.

The TUHC Outpatient Physical and Occupational Therapy clinic provides services to children and adults. In addition to providing care to patients with orthopedic and neurologic conditions, we have several specialty areas. The Occupational Therapy clinic is a specialty clinic for hand rehabilitation, with patients seen by a Certified Hand Therapist. Other specialty services include Vestibular rehabilitation, Lymphedema management, incontinence management and wound care. The therapists see a general population of patients with orthopedic and neurologic conditions.

The department is an excellent place to work. The concept of team is epitomized by the people who work here. The environment supports clinical growth, learning from each other and through ongoing seminars and in-services. As a hospital based outpatient clinic, the therapists have a close working relationship with the medical staff.

Therapy Services—Speech Pathology Department
Communication and swallowing disorders are one of the nation’s leading forms of disabilities. More than 24 million people in the United States suffer from some form of speech, language, cognitive, or swallowing problem. The Tulane University Hospital & Clinic Speech Pathology Department provides state-of-the-art diagnostic evaluations and treatment for pediatric through adult patients. All staff members are nationally certified and are supported by the unsurpassed expertise of the more than 300 specialists and sub-specialists of the Tulane Medical staff. The speech Pathology department offers comprehensive diagnostic assessment and treatment for:

• Pediatric and Adult Speech production, language and cognitive/communication deficits
• Adult swallowing disorders
• Pediatric feeding and swallowing disorders
• Use of one way speaking valves for tracheostomy and ventilator dependent patients

Populations typically serviced include infants, children, and adults with diagnosis of traumatic brain injury, stroke, spinal cord injury, neuromuscular disease, brain tumor resection, craniofacial abnormalities, tracheostomy or ventilator dependent, autism, developmental delay, speech and language disorder, or laryngeal pathology.

TISM
The Tulane Institute of Sports Medicine (TISM) is a state of the art “one-stop” center of excellence for healthy orthopedic and sports injury care. The Institute combines orthopedics, sports medicine, radiology (including MRI), rehabilitation, and the only clinically oriented biomechanics lab on the Gulf Coast in one setting. All emergency sports injuries are seen in a timely fashion with aggressive care to return our patients to their sport (or work) as quickly and safely as possible. This occurs because of the close knit team of physicians, physician’s assistants, radiologists, nurses, physical therapists, athletic trainers and bio mechanists. Because TISM combines both research, education and sound evidence based medicine to every patient, our patients and staff are excited about the care delivered in this friendly environment. Tulane Hospital for Children
The 6th floor is a 32-bed unit for children 0-21 years who require highly specialized medical and nursing care. Space constraints currently allow us only 19 private rooms that are designed for those children who require isolation to protect either themselves or others.

I. Child Life Services
A Child Life Specialist is a professional who has been trained to help children with hospitalization. The Child Life Specialist is there to help the children understand and learn about their illness and medical procedures. The Child Life offers the children at Tulane Hospital for Children the opportunity to socialize and participate in organized therapeutic activities.

II. Pediatric Intensive Care Unit (PICU)
At Tulane Hospital for Children, our goal is to provide the highest level of medical and nursing care available for infants, children, and adolescents. Our first responsibility is to the child, and, as we work towards creating a family centered approach to caring for children, we want to include the parents in the care of the child any way we can.

The Pediatric Intensive Care Unit provides highly specialized care and comprehensive treatment to a wide range of critically ill or injured children from birth to 21 years of age. The PICU is a 14 bed unit.

Tulane Hospital for Children has some of the country’s finest doctors and nurses specializing in all aspects of pediatric care. Our pediatric intensive care physicians are board certified in pediatric critical care and work closely with other Tulane Hospital for Children specialists, including cardiologists, cardiovascular surgeons, nephrologists, pulmonologists, transplant surgeons, and many more. Our nurses and respiratory therapists are also certified in the care of critically ill infants, children and adolescents.

III. Neonatal Intensive Care Unit—2nd Floor Tulane-Lakeside Campus
The NICU is located on the fourth floor. The Neonatal Intensive Care Unit (NICU) of Tulane Hospital for Children was established in 1977. The NICU provides highly specialized care and comprehensive treatment to a wide range of critically ill neonates. The NICU consists of a 16-bed NICU Level 3 regional unit with two isolation rooms and a combined Level II, III and IIIR.
Tulane Hospital for Children has some of the country’s finest doctors specializing in all aspects of neonatal care. The neonatologists providing care in these units work closely with other Tulane Hospital for Children specialists. An important goal in patient care is to prevent the spread of infection to patients, visitors and staff.

Our critical care teams consist of residents, interns, and neonatal nurse practitioners. The critical care team cares for all the patients in the NICU.

Residents are physicians who are receiving advanced training in the hospital. They participate in baby care under the direction of the neonatologist. Fellows are physicians who have completed their residency and are now training in a special area of pediatrics. Consulting physicians are experts in a specific area of pediatrics.

A neonatal nurse practitioner is a registered nurse who has become certified in advanced care of ill newborns. A registered nurse is always assigned to care for babies. One nurse will be assigned one to two patients depending on the level of your baby’s illness.

A charge nurse is assigned on every shift. The charge nurse oversees that the unit runs smoothly during the shift and helps other nurses with duties. The NICU coordinator supervises the nursing care provided by the unit staff. The clinical coordinator is also available on duty. The NICU case managers work in partnership with the parents and medical team to develop discharge plans. They also assist families in working through insurance issues.

A lactation consultant is also available to assist those mothers who wish to breast feed their babies. A pumping area, freezer and supplies are available to make breast feeding a positive experience during a difficult time. Other specialized team members, such as nutritionists, physical therapists, and occupational therapists, may also be involved.
